She was born to parents restaurant magnates Reynaldo and Cecilia Ongpauco, on February 14, 1985. She is the youngest of the six siblings that include a brother (Miguel) and four sisters (Lisa, Marjorie, Michelle and Camille). Evangelista belongs to the Ongpauco clan of Barrio Fiesta fame. Rod Ongpauco is her uncle, who is also the founder of the famous Barrio Fiesta chain of restaurants. Her father, Reynaldo Ongpauco is the man responsible for laying the foundation of many branches of the Barrio Fiesta restaurants in San Francisco. In a way, Evangelista was born with a silver spoon and she was bought up in the United States. She grew up and studied at Audabon School in San Francisco, California in the United States of America. Once she completed her studies, her family moved back to the Philippines in her early years and transferred at Colegio San Agustin-Makati in Makati City. When she entered into show business, she continued her high school at ABS-CBN`s Distance Learning Center (DLC).
Popularity was never far away for Evangelista and today, she is looked upon as one of the most popular and multi-talented personality. Heart is a true Filipina actress, singer, VJ and model. It all started when an agent spotted her during one of her shopping day outs with Camille, her elder sister. Evangelista was soon signed for Star Magic, which was formerly known as the Talent Center. She was paired with John Prats for a show called "G-Mik". There seemed to be an instant hit because the pair sizzled on-screen and brought their respective characters to life. John Prats and Evangelista starred in many more movies together like "Trip", "My First Romance", "Ang Tanging Ina" and the popular teen show "Berks". Soon after this show, the off-screen as well as the on-screen romance between Evangelista and Prats fell apart.
Heart was soon ready for another shot at fame and landed a role in a primetime soap opera called Hiram. She was paired opposite Geoff Eigenmann in "Hiram", which also had a long star cast including Kris Aquino , John Estrada, Dina Bonnevie, and Anne Curtis . "Hiram" turned out to be a milestone for Evangelista as she earned the Star Awards Best Actress nomination at the young age of 20. Multi-talented and a winner all the way, Evangelista has also won the award for best new female personality for Star TV as well as in Star Movie awards. Not to be left behind in music, Evangelista won the Texter Listeners Awards with her single "Love Has Come My Way" from the Album Himig Handog Love Songs relased in 2002. In 2003, she releases another album called "Heart".
Evangelista also won the best new female personality (Best New Movie Actress and Best New TV Actress) awards from Star Awards for Movies and Star Awards for TV. She also won Texter`s and Listener`s Choice at the Himig Handog Awards for her single "Love Has Come My Way". Heart also released the single "Tell Me" in Australia. It peaked at number 18 on the charts. The top twenty single is being re-released on a greatest hits package of Australian pop hits.
Evangelista worked on and off for three years (2005-2008) and focused mainly on hosting gigs, starring on a few Maalaala Mo Kaya and Your Song episodes, and giving time to her foundation Heart Can. In early 2007, she renewed her ties with her mother station ABS-CBN and was under a per project basis with her manager Angeli Pangilinan Valenciano. She finished a sineserye (TV series based on a film) for the station, the remake of the movie Hiram na Mukha, and appeared briefly on Pangarap Na Bituin. She also topbilled the reformatted version of Your Song under the title "Muntik Na Kitang Minahal".
She was also offered to do another album under Star Records and return to ASAP. She was supposed to play again her character in Ang Tanging Ina Ninyong Lahat as one of Ai Ai de las Alas` daughters. But all of these she declined because she chose to transfer to GMA Network due to her father`s decision.
She announced her sudden transfer to ABS-CBN`s rival network, GMA Network, last April 23, 2008. She signed up a two-year exclusive contract under her new manager Annabelle Rama and was paired with Richard Gutierrez. Their first action-teledrama together was on GMA`s series Codename: Asero. Evangelista is now co-hosting on SOP Rules.
Evangelista played the title role in Ay Ayeng, a film on the indigenous Ifugao life in Mountain Province, which premiered in Hawaii, on July 14, 2008. The movie was adapted from "Maestra Ester," a novel by Abdon Noviza and Lica Lazaro.
